NOTICE TO GROVE HILL HEALTHCARE PATIENTS
This practice serves all patients regardless of
inability to pay. Discounts for essential services are
offered depending upon family size and income.
You may apply for a discount at the front desk.
INCOME BASED REQUIREMENTS
When you apply for our sliding fee program, there a few things you will want to keep in mind:
1. Due to discounted rate, payment is expected at the time of service.
2. Sliding fee will be approved for one year. You must reapply yearly and bring proof of income. Proof of income may include tax returns, W2 forms, current paycheck stubs, SSI statement, and a copy of any court ordered child support.
3. If your income increases or decreases, notify the clinic immediately to allow for modification of your rate, if appropriate.
4. All unpaid accounts are subjected to our collection policy.
*All primary care services are covered under Grove Hill Healthcare’s sliding fee program.
